Overview

Pancreatic cancer is a malignancy arising from the exocrine or, less often, endocrine tissue of the pancreas, with pancreatic ductal adenocarcinoma accounting for the large majority of cases. It is characterized by aggressive biology, early local invasion of adjacent vasculature and bile ducts, and a propensity for distant spread, which together explain its poor prognosis. The clinical course is shaped by late presentation, because early disease is frequently asymptomatic and symptoms such as obstructive jaundice, weight loss, abdominal or back pain, and new-onset diabetes often emerge only once the tumour is advanced. Diagnostic evaluation integrates cross-sectional imaging, tissue sampling, and serum markers, and the comparative value of these markers is an active question: bilirubin and carcinoembryonic antigen, for example, may carry predictive weight alongside or beyond CA 19-9 at initial presentation. Atypical first presentations, including massive upper gastrointestinal haemorrhage from advanced adenocarcinoma, underscore the disease's clinical heterogeneity. Management combines surgical resection in operable cases with chemotherapy and radiotherapy, while supportive concerns such as cancer cachexia and nutritional optimization are integral to care.
