Overview
Atherosclerosis is a chronic, progressive disease of the arterial wall characterised by the accumulation of lipid, inflammatory cells, smooth-muscle cells, and fibrous tissue into plaques that thicken and stiffen the intima and narrow the lumen. Its pathogenesis begins with endothelial dysfunction and the retention and oxidation of low-density lipoprotein, followed by monocyte recruitment, foam-cell formation, and an inflammatory cascade that drives plaque growth; advanced lesions may rupture and trigger thrombosis. Atherosclerosis is the principal substrate of coronary artery disease, ischaemic stroke, and peripheral arterial disease, and contributes to aneurysm formation. Recognised risk factors include dyslipidaemia, hypertension, diabetes, oxidative stress, and lifestyle exposures, while subclinical disease is assessed through markers such as carotid intima-media thickness and coronary artery calcification. Research spans the molecular and genetic determinants of susceptibility, including coronary-disease-associated polymorphisms and microRNA regulation, endothelial function, lipid metabolism, and the interplay with metabolic and inflammatory conditions.
