Overview

Coronary heart disease, also termed coronary artery disease, is the narrowing or obstruction of the coronary arteries that supply the myocardium, most often through atherosclerosis, leading to a mismatch between myocardial oxygen demand and supply. The accumulation of lipid-rich, inflammatory atherosclerotic plaque within the arterial wall, and its potential rupture and thrombosis, produces the clinical spectrum from stable angina to acute coronary syndromes, myocardial infarction and sudden cardiac death. It is a leading cause of morbidity and mortality and is driven by modifiable risk factors including dyslipidaemia, hypertension, smoking, diabetes and physical inactivity, alongside genetic predisposition. Mechanistic understanding centres on lipoprotein deposition, endothelial dysfunction and plaque biology, while diagnosis and management combine risk-factor control, pharmacological therapy and revascularisation such as percutaneous coronary intervention. Research collected under this term reflects these dimensions: outcomes of percutaneous coronary intervention in elderly patients with ischaemic heart disease, gene-polymorphism associations with coronary heart disease, modelling of low-density-lipoprotein deposition in coronary arteries, effects of smoking and vaping, heart failure in primary care, microRNA strategies in ischaemic heart disease, and chronic-disease risk-factor prevalence. The peer-reviewed literature in this area spans atherosclerotic mechanisms, lipid biology, risk factors, and the diagnosis, intervention and management of coronary artery disease and its ischaemic consequences.
