Overview
Blood cholesterol is the concentration of cholesterol circulating in the bloodstream, transported within lipoprotein particles because the molecule is insoluble in plasma. Cholesterol is an essential lipid that forms a structural component of cell membranes and serves as a precursor for steroid hormones, bile acids, and vitamin D. It is carried chiefly by low-density lipoprotein, which delivers cholesterol to peripheral tissues, and high-density lipoprotein, which mediates reverse transport to the liver; the balance among these fractions, together with triglycerides, defines the lipid profile. Elevated low-density lipoprotein and total cholesterol promote atherogenesis through deposition within arterial walls, driving cardiovascular disease including coronary heart disease, myocardial infarction, and stroke, and clustering with obesity, central adiposity, insulin resistance, and metabolic syndrome to compound cardiometabolic risk. Assessment of blood lipids supports risk stratification, exemplified by composite scores such as the Framingham risk estimate, and informs preventive strategy. Modifiable determinants include diet, body composition, and physical activity, and dietary and lifestyle interventions can favorably alter lipid concentrations and related homocysteine and metabolic markers.
